COVID-19 Family Vaccination Clinic Scheduling Form

The Erie County Department of Health (ECDOH) is organizing a COVID-19 vaccine clinic with a family focus on Wednesday, October 29th, 2025. COVID-19 vaccines will be available for children ages 6 months and older, as well as for any adult members of their immediate household. For additional clinic dates and locations, please go to erie.gov/vax.

Schedule

Walk-ins are welcome, but scheduled appointments are strongly recommended. Please sign up for a time slot below. Each individual should have their own time slot, so please submit a form for each individual being scheduled.

Who is eligible?

  • Any child age 6 months-17 years, and any adults in their immediate family/household.
  • Prescriptions are not needed.
  • Please contact your doctor with specific questions about your health and risks for COVID-19 illness.
  • Immunocompromised individuals should have a conversation with their primary care provider about additional doses needed.
  • People without insurance will be able to receive a vaccine at no cost.

What to bring?

  • Patients should bring their health insurance card (if applicable) and photo ID.
  • Minors under the age of 18 must be accompanied by a parent or legal guardian.

Note: Erie County is following the COVID-19 vaccine recommendations put forth in September 2025 by the Northeast Public Health Collaborative, of which New York State is a part. These guidelines are in line with American Academy of Pediatrics (AAP), the American College of Obstetricians and Gynecologists, and the American Academy of Family Physicians science-based recommendations.
