Clinic Schedule and Registration
- Click on the links in the calendar below to schedule an appointment (walk-ins are also accepted) and to see what vaccinations are offered at that clinic.
- If this is not your first vaccination, then please bring your Vaccination Record Card with you to the vaccination site.
- Use CDC’s COVID-19 booster tool to learn if and when you can get boosters to stay up to date with your COVID-19 vaccines.
- The 3rd and 4th dose are NOT booster doses. They are for individuals who are moderately to severely immunocompromised and are available, at all of the clinics listed in the calendar below, on a walk-in only basis. Please talk to your healthcare provider if you have additional questions.
Consent for Minors: Individuals under the age of 18 years old must have a parent/legal guardian give consent. This consent can be in-person, verbal by phone or in writing by using the COVID-19 Consent for Vaccination of Minors Form for Ages 6 Months - 11 Years and the COVID-19 Consent for Vaccination of Minors Form for Ages 12-17 Years.
Vax Visit In-Home Vaccination Program: If you prefer, you can be vaccinated in your home through our Vax Visit Program. This service is free and available to Erie County residents ages 6 months and older. Please call 716-858-2929 to register and for additional information.
