Hours & Appointments

  • Immunizations are available by appointment only. Call (716) 858-7687 to schedule an appointment.
  • 2nd and 4th Fridays of every month
  • 8:30 am to 11:30 am

Things to Bring with You

  • All immunization records.
  • Documentation from school showing requirement that student needs to be immunized (when applicable).
  • Parent or legal guardian must be with individuals under age 18 years old. 

Vaccines Available & Cost

  • See below chart for cost.  Payment is due at the time of your visit. Payment methods accepted include cash, check, and credit card (Visa, Master Card, Discover, American Express; a nominal fee applied for this method of payment). 
  • We do not accept insurance.
  • We do not perform titers.

Vaccines for Children Program (VFC)

Federally funded program that provides vaccines at no cost to children who might not otherwise be vaccinated because of inability to pay. 

VFC eligibility: 

  • A child is eligible for the VFC Program if he or she is younger than 19 years of age and is one of the following:
    • Medicaid-eligible
    • Uninsured
    • Underinsured
    • American Indian or Alaska Native

NYS Vaccines for Adults Program (VFA)

State funded program for adults to receive publicly purchased vaccine in NYS. 

VFA eligibility:

  • Uninsured adults ages 19 years and older
  • Underinsured adults ages 19 years and older (i.e., health insurance does not cover the costs of the vaccine to be administered)

Post-secondary students

  • Students of any age that are enrolled in or entering a post-secondary institution in New York State.
  • Fully insured post-secondary students are only eligible to receive measles, mumps, and rubella (MMR) vaccine through the VFA program.
