Booster and Third Dose of COVID Vaccine

Booster Dose:

  • The ECDOH will vaccinate anyone age 16 and older with a COVID-19 booster dose as long as it has been 6 months after their 2nd dose of Pfizer or Moderna or 2 months after their single dose of J&J). We encourage people to review the recommended booster eligibility criteria, evaluate their risk factors and talk to their own physician with questions.
  • CDC recommendations now allow individuals to choose which type of vaccine they receive as a booster dose.
  • Individuals under 18 years of age must have a parent/legal guardian give consent (In-person, verbal by phone or in writing by using the COVID-19 Consent for Vaccination of Minors Form for Ages 12-17).
  • If you require a special accommodation, please contact the Office for People with Disabilities at 716-858-6215 at least 48 hours prior to your appointment.

Click on the links below to schedule an appointment.  Appointments are highly recommended.  Walk-ins are accepted during all operating hours at the Buffalo Homecare locations and during the last 2 hours at the other sites listed below.  Please bring your Vaccination Record Card with you to the vaccination site.

3rd Dose:

  • The 3rd dose is NOT a booster dose. The 3rd dose is for individuals, ages 12 and older, who are moderately to severely immunocompromised (weakened immune system).
  • These people are especially vulnerable to COVID-19 and may not build the same level of immunity to two-dose vaccine series compared to people who are not immunocompromised.
  • See the COVID-19 Vaccine 3rd Dose Qualification Form for qualifications and talk to your healthcare provider if you have additional questions.
  • Individuals 12-17 years old must have a parent/legal guardian give consent (In-person, verbal by phone or in writing by using the COVID-19 Consent for Vaccination of Minors Form for Ages 12-17).
  • CDC recommends that eligible individuals receive a 3rd dose of the Pfizer or Moderna 28 days after their second dose.
  • If you require a special accommodation, please contact the Office for People with Disabilities at 716-858-6215 at least 48 hours prior to your appointment.
