CDC now recommends that children between the ages of 5 and 11 years receive the Pfizer Pediatric COVID-19 Vaccine. More information can be found here. Individuals 5-11 years old must have a parent/legal guardian give consent (In-person, verbal by phone or in writing by using the COVID-19 Consent for Vaccination of Minors Form for Ages 5-11 Years.
Children 5 through 11 years old will receive a separate vaccine formulation denoted with an orange cap that has one-third the dose given to adolescents and adults, and will receive the vaccine with a smaller needle. See the description of the differences in the adult/adolescent formulation and pediatric formulation for more information.
Appointments for ages 5-11 are REQUIRED. Click on the links below to schedule an appointment. Dose 1, 2, 3 and Booster for Pfizer and Moderna are also available at the following locations on a walk-in only basis.
