- All individuals 12 years of age and older are eligible for Pfizer Vaccine. Individuals 12-17 years old must have a parent/legal guardian give consent (In-person, verbal by phone or in writing by using the COVID-19 Consent for Vaccination of Minors Form for Ages 12-17).
- All individuals 18 years of age and older are eligible for Moderna Vaccine.
- The Pfizer and Moderna vaccines require two doses. You should schedule your second dose 21 days (Pfizer) or 28 days (Moderna) after the first dose. If you are unable to do that be sure to get the second dose no matter how much time has passed.
