DEPARTMENT: Senior Services

LOCATION: 95 Franklin Street

SHIFT: 8:30 a.m.- 4:30 p.m.

TITLE OF POSITION: Case Manager-Senior Services

POSITION NUMBER: 51009766

SALARY: $22.37 per Hour (CSEA Job Grade:VII)

*Please note that this position is a competitive civil service title and will require a civil service examination in the future. After the exam, selection for permanent appointment(s) will be made from reachable candidate(s) on the eligible list according to the NYS Civil Service Rule of Three.

The work involves responsibility for guiding elderly people and their families through the service system. The work includes provision of information and assistance; performing pre-screening (or intake) functions; and through assessment of financial, health, mental health and socio-emotional strengths and deficits, in order to develop and implement case plans. Services are provided to avoid unnecessary institutionalization of elderly residents of Erie County. Work is performed under the direct supervision of a Senior Case Manager, Social Case Supervisor or and administrative employee of higher rank. Supervision may be exercised over lower ranking employees. Does related work as required.

TYPICAL WORK ACTIVITIES:

Provides information and assistance on services to the elderly, their families and other service providers on the telephone, in person and through community forums;

Screens calls at the point of intake to determine the need for case management services;

Assesses elderly clients in their homes, and develops case plan to meet needs;

Maintains client data base in required software;

Implements case plan and revises it as necessary;

Advocates on behalf of clients with formal and informal support systems;

Attends meetings and case conferences with other agencies relating to the provision of services to the elderly;

Assesses clients for home health care services and monitors appropriateness of care;

Maintains daily activity sheets, case records, and other documentation, submitting written reports as required;

Generates and responds to correspondence, using appropriate software;

Consults with supervision on difficult cases;

May enter various courts to represent the client, thereby protecting his/her person and/or property;

May be involved in the planning and implementation of formal or informal financial management for the elderly;

Intervenes in crisis situations, including medical, environmental, mental health and utility emergencies;

May assist with transportation of elderly clients, including use of agency vehicles, in an emergency or occasional situation.

MINIMUM QUALIFICATIONS:

Possession of a Bachelor’s degree.

SPECIAL REQUIREMENT:

Possession of a New York State driver's license at time of appointment. Since these positions require substantial travel, candidates must demonstrate their ability to meet the transportation needs of the job in carrying out their responsibility in any location to which travel is required, including some locations not accessible by public transportation.
