Title of Position: Case Manager- Senior Services (HELP Program) - two (2) vacancies
Shift: Full time
Monday- Friday 8:30am- 4:30pm
Starting Annual Salary: $23.409/hr. or $47,942/yr
Who we are looking for:
Erie County is looking for a resourceful, dependable, and hard-working individual for the position of Case Manager within the department of Senior Services. If you like routine paperwork with an assortment of diverse situations, then this job is for you! We are continually expanding and adapting our services and striving to improve how we deliver them to the older adults in our community.
A Senior Services Case Manager provides assessments and service monitoring to folks who are 60 + and their caregivers. This entry level position offers a variety of experiences related to all aspects of aging –from conducting comprehensive assessments to learning next steps when concerns are revealed as well as, developing person centered care plans. The work also includes basic benefit application assistance and providing linkages to local community resources. On-the-job training included!
What qualities are we looking for?
- Good knowledge of current principles, practices and techniques of social work and case management and good knowledge of the principles of social work as related to the 60+ population;
- Ability to establish and maintain relationships with elderly clients, families, and service providers; ability to interview, assess, establish and implement case plans on behalf of clients;

- Capable of performing the essential functions of the position with or without reasonable accommodation.
What are the typical work activities?
- Provides information and assistance on services to adults ages 60+, their families and other service providers on the telephone, in person and through community forums;
- Screens calls at the point of intake to determine the need for case management services;
- Assesses clients in their homes, and develops case plan to meet needs;
- Maintains client data base in required software;
- Implements case plan and revises it as necessary;
- Advocates on behalf of clients with formal and informal support systems;
- Attends meetings and case conferences with other agencies relating to the provision of services to the clients;
- Assesses clients for home health care services and monitors appropriateness of care;
- Maintains daily activity sheets, case records, and other documentation, submitting written reports as required;
- Generates and responds to correspondence, using appropriate software;
- Consults with supervision on difficult cases;
Minimum qualifications:
Possession of a Bachelor’s degree.
Special Requirements:
Eligibility for a New York State driver license at the time of application. Possession of the license and use of a private automobile required at time of appointment
