The work involves assisting in providing professional counseling and casework services to participants of the Medication Assisted Treatment (MAT) program. The incumbent provides assistance to patients and their families relative to medically related social and emotional problems. The incumbent serves as a member of an interdisciplinary health care team which performs a variety of professional responsibilities. Incumbents consult and cooperate with other professionals and agency personnel to aid them in recognizing the significant social components of health care and understanding their impact on patients and their families. Work is performed under the supervision of the Director of Correctional Health Services for clinical matters and the Chief of Community Reintegration for non-clinical duties. Does related work as required.


TYPICAL WORK ACTIVITIES:
•    Interviews patients to obtain social history relevant to medical problems.
•    Arranges for post discharge care facilities and assists patients and families in handling and accepting nursing home placement;
•    Assists patients in securing appliances required through agencies and rentals;
•    Compiles medical-social history summarizations and statistical reports related to social work activity;
•    Confers with hospital personnel, as necessary, on behalf of patients; arranges for transportation;
•    Collects and revises information on community health and welfare resources;
•    Maintain current knowledge of code requirements as they apply to social work in long term care;
•    Participates in the selection of patients to be admitted to the MAT program;
•    Informs residents and/or responsible others of residents' rights and responsibilities; Completes psychosocial assessment;
•    Maintains contact with residents and families to provide information and emotional support;
•    Participate in interdisciplinary, administrative, staff, policy and other meetings deemed appropriate; Assists with presentation of in-service training programs dealing with psychosocial issues;
•    Assess residents' discharge potential and serve as discharge coordinator; Maintain appropriate resident files;
•    Compiles reports and statistical information.
*Please note that this position is a competitive civil service title and selection for permanent appointment(s) will be made from reachable candidate(s) on the eligible list according to the NYS Civil Service Rule of Three.
