TITLE OF POSITION: Forensic MH Community Discharge Planner*
TYPE OF POSITION: *Provisional (permanency pending civil service exam)
EXAM TYPE: Open Competitive (open to the public, exam required)
LOCATION: 120 West Eagle Street, Buffalo
SHIFT: Monday – Friday, 8:00am – 4:00pm
SALARY: Starts at $53,972 Annually
POSTING DATES: February 29, 2024 – March 10, 2024
* The person hired for this position will have to take a civil service examination at a future date and attain a score that is reachable under NYS civil service law. *
This position facilitates treatment linkages to appropriate mental disability services in the community. In addition, this position provides the case management and support services necessary to ensure successful and sustainable release back into the community including the initial community engagement, initial entitlement planning and linkage to other health and human service supports. Work is performed under the general supervision of an administrative employee of higher rank. The position performs related work as required.
MINIMUM QUALIFICATIONS:
Possession of a Bachelor’s Degree in Social Work, Psychology, Criminology, or Psychiatric Nursing. One-year experience providing treatment, case management or rehabilitation services to individuals with serious mental health disorders and co- occurring addictions disorders.
Candidates must be legal residents of Erie County at the time of Provisional appointment, for at least one month immediately preceding the date of the written test, AND at the time of Permanent appointment.
