Who we are looking for?

Erie County is looking for a resourceful, dependable, and hard-working individual for the position of Long-Term Care Coordinator. The Long-Term Care Coordinator is responsible for the management and coordination of multiple units including: case management, NY Connects, HIICAP, Senior HEAP, caregiver services and transportation. Efficient time management, flexibility, ability to multitask on various projects and initiates, ability to manage a large team, and positive attitude are some of the key characteristics that are critical to this position.  

What qualities are we looking for? 

• Good knowledge of current principles, practices and techniques of social work and case management and good knowledge of the principles of social work as related to the 60+ population; 

• Ability to establish and maintain relationships with elderly clients, families and service providers; ability to interview, assess, establish and implement case plans on behalf of clients;

What are the typical work activities? 

• Plans, organizes, directs, and coordinates the various functions comprising the long-term care program designed for the community; 

• Assists partnering departments and agencies with the formulation of policies, procedures, and objectives in providing long term care services; 

• Coordinates community-wide public education on Point of Entry function, access, and philosophies of personal choice and effectiveness; 

• Conducts and/or assists with the training of agency personnel on Erie County NY Connects function, access, and resources related to long term care; 

• Executes and monitors, and documents milestones of the Point of Entry agreement with state agencies; 

• Plans, assists with the development of, and evaluates programs providing health and social programs for long term care consumers including case management, HIICAP, caregiver services, HEAP, and transportation;

 • Monitors the effectiveness of long-term care services as they relate to the goals of the Point of Entry program; 

• Works with Erie County NY Connects intake staff in carrying out their duties and responsibilities for the long-term care program; Shift: 

• Maintains the web-based/network-based resource directories of the Point of Entry designed for public and professional use; 

• Participates in ongoing quality improvement of Point of Entry function. 

Minimum qualifications: 

A.) Graduation from a regionally accredited or New York State registered college or university with a Master's degree and two (2) years of administrative experience in a human services agency which includes serving an adult population; or 

B.) Graduation from a regionally accredited or New York State registered college or university with a Bachelor's degree and four (4) years of administrative experience in a human services agency which includes serving an adult population; or 

C.) Graduation from a certified nursing program with an RN license, currently maintained, and four (4) years of administrative experience in a health or human services agency which includes serving an adult population; or 

D.) An equivalent combination of training and experience as defined by the limits of(A), (B), and (C).
