Title of Position: Senior Case Manager
Shift: Full Time Provisional – Required to take recently announced civil service exam (see #77-845 applications due 1.24.2024)
Monday- Friday 8:30am- 4:30pm
Starting Salary: $28.064/hour , $58,372 Annually, Job Group 9
Who we are looking for:
Erie County is looking for an accommodating, driven, and dependable individual for the position of Senior Case Manager within the department of Senior Services. A Senior Case Manager is skilled in both direct case management and the training, supervision, and guidance of lower ranking employees. They offer in-services, and provide input to higher ranking staff who develop policy. Senior Case Managers assist in the implementation of policy and adherence established case management standards. Does related work as required.

What are the typical work activities?
- Assess or assists in the assessment of elderly clients to determine appropriate plans of care
- Implements, monitors (through case conferences with case managers and/or visits to community agencies) and evaluates the effectiveness of plans of care including but not limited to: referring people to private and public agencies; planning or implementing financial management for clients; provide final approval for implementation of case management services
- Develops and implements training and in-services for staff and community groups
- Investigates and resolves complaints from clients, families, and other parties which affect the delivery and effectiveness of service
- Assists with implementation of single point of entry and/or NY Connects
- Assists with implementation of mental health interventions of system development
- Implements standards set forth by funding streams
- Provides input for the evaluation of lower ranking employees as well as guidance
- Obtains information and provides to higher ranking employees
- Represents the unit/department through attendance and provision of information at committee meetings, community forums, or other appropriate venues
- Maintains records, submits reports, handles correspondence in accordance with accepted standards, collects data to evaluate performance measure, quality improvement and outcomes all in an appropriate time frame
- Learns and uses computer software as required
- Provides back-up coverage for lower ranking employees as needed
- Possession of a Bachelor's degree and two (2) years of experience in a position related to a community organization or social agency in the human services area.*
SPECIAL REQUIREMENTS:
- Eligibility for New York State driver's license required at time of application. Possession of the license required at time of appointment. Since these positions require substantial travel, candidates must demonstrate their ability to meet the transportation needs of the job in carrying out their responsibility in any location to which travel is required, including some locations not accessible by public transportation.
- Applicants for designated positions in the Department of Senior Services may be subjected to a background check. Inquiries on current employees shall be made annually and prior to promotion but no more often than once in any six month period (N.Y. Social Services Law 424-A.) Candidates/applicants with conditional offers of employment may be required to submit the necessary fee(s) for background screening and inquiries shall be completed prior to employment hiring/start date. Refusal to sign the necessary clearance forms, submit the required associated fees and/or participate in the review process shall be cause for an automatic non-selection. In addition, inquiry responses are subject to evaluation and may result in disqualification pursuant to Section 50 of the New York State Civil Service Law.
- Applicants for designated positions in the Department of Senior Services that may have access to federal tax information to perform his/her job duties shall be subject to a suitability background check investigation, including FBI fingerprinting, local law enforcement check, and verification check of citizenship/residency. A criminal record does not necessarily disqualify applicants from employment, rather an individualized determination will be made. The cost of the background check will be assumed by the Department of Senior Services. NOTE 1: *HUMAN SERVICES: Dealing with the social, mental and emotional needs of the people. NOTE 2: Verifiable part-time and/or volunteer experience will be pro-rated toward meeting full-time experience requirements.
