Erie County 2025-2030 CHA and CHIP Community Partner Meetings and Resources

Every six years, counties across New York State are required to complete a Community Health Assessment (CHA) and a Community Health Improvement Plan (CHIP). As part of this important effort, community engagement is not only encouraged, it is essential.

Over the past year, the Erie County Office of Health Equity (ECOHE), in partnership with Catholic Health, Kaleida Health, and ECMC, conducted two surveys—one with community members and another with health care providers—to identify the most pressing health concerns in our region. To complement these surveys, we also held interviews and focus groups with several community groups and health care providers. These conversations offered valuable insights into lived experiences and community-specific challenges.
