Fee for Service.

Perspective patients are required to fill out an application and proceed through a screening appointment.  Application and screening do not guarantee acceptance.  Request a patient application packet from the patient admissions office at (716)829-2732.  Complete forms and return. There is a fee unless Medicaid active. DO NOT SEND CASH.

Patients who do not qualify to participate in the clinical educational program will be notified in writing.  We regret that all patients screened cannot be accepted for dental care.

The fees charged are substantially less than the cost of care from a private dentist and treatment progresses more slowly.  The lower fees are offered to compensate for the longer treatment time and more frequent visits.  Your estimated fees will be presented to you along with a treatment plan before any substantial treatment is begun; however, payment is required at time of service.  It may become necessary to refer some or all of your treatment to one of the post-graduate or specialized clinics. Fees for treatment by dentists in the advanced clinics are higher than those offered in the student dental clinics (pre-doctoral), because the providers are licensed dentists working either toward a specialized degree or gaining additional experience in a general dentistry setting.  You will be informed if all or part of your dental care requires referral to one of those clinics.

Please note: this is not a comprehensive list.  Contact providers in your area to inquire about dental services.
