Request Paper Copies of Health Equity Publications

Use this form to request paper copies of the Office of Health Equity publications for your community-based organization, block club, or neighborhood group. Publications are released periodically and will be mailed out to the address you provide below. You may request to subscribe to all new publications as they become available or request a one-time order of a particular edition.
