Hospice Buffalo cares for seriously ill people with a prognosis of 6 months or less, who are not receiving aggressive or curative treatments. Our patients receive comprehensive care from an interdisciplinary team that addresses the medical needs of the individual, as well as the psychosocial, emotional, and spiritual needs of both the patient and their family. Home Connections, a home-based palliative care program, specializes in providing relief of pain, symptoms, and stress experienced by those with serious illness. Essential Care for Children –a home-based palliative care program, provides coordination of medical services, as well as psycho-social, emotional, and spiritual support to seriously ill children and their families. Specially trained experts work closely with the child’s medical team to improve symptom management and care coordination to avoid unnecessary ER visits and hospitalizations. Wilson Support Center – provides community grief support and education that is sensitive, compassionate, and responsive to the needs of our Western New York Community. Support is provided by licensed counselors and customized for each person, because we recognize everyone copes with grief differently.
