Health Force can help seniors and the disabled in 4 different means. First as a NYS Medicaid home health care service provider. Secondly as a care provider under many Medicaid-funded programs such as Consumer Directed Personal Aide Program (CDPAP). Thirdly, working with Headway providing up to 24/7 care through the NYS Traumatic Brain Injury (TBI) Waiver and the Nursing Home Transition and Diversion (NHTD) Waiver. Lastly as we accept private insurance, workers compensation and self-pay are also accepted. Services being offered include service coordination, independent living skills training, positive behavioral interventions, community integration counseling, home and community support services, and structured day program services.
