Venture Forthe is a Licensed Home Care Services Agency dedicated to helping the elderly and those with disabilities live a quality life in the community as an alternative to a nursing home. Venture Forthe is a NYS Medicaid home health care service provider. We proudly provide care under many Medicaid-funded programs such as Consumer Directed Personal Aide Program (CDPAP), traditional homecare services Personal Certified Aides (PCA) as well as caring for those in need of up to 24/7 care through the NYS Traumatic Brain Injury (TBI) Waiver and the Nursing Home Transition and Diversion (NHTD) Waiver. Private insurance, workers compensation and self-pay are also accepted. Services being offered include service coordination, independent living skills training, positive behavioral interventions, community integration counseling, home and community support services, and structured day program services.
