We provide case management services to support older adults (age 60+) with their health and daily living needs.
Our caring team works with each older adult to:
Understand their needs Example: A case manager meets with Mrs. Johnson, who is having trouble managing her medications and getting to doctor appointments.
Create a care plan Example: Together, they make a plan to set up a medication reminder system and arrange rides to her medical visits.
Connect them to services Example: Mr. Lewis is connected with Meals on Wheels, a senior center near his home, and help applying for SNAP benefits.
Check in regularly (at least every two months) Example: A case manager calls or visits to see how things are going and if any new support is needed.
Make updates as needed Example: When Mrs. Johnson’s needs change, her care plan is updated to include in-home health services.
Our goal is to help seniors stay independent, safe, and connected to the support they need.
Case Managers are staff who help you find the right answers, are knowledgeable about resources regarding all aspects of aging, they help sort through all possible options and will help apply for benefits and locate services in the community. Funding for home care, home-delivered meals, emergency buttons, weatherization, and financial benefits are just some of the wide range of services available.
