The ALS Association Upstate New York Chapter is dedicated to providing resources and services to improve the quality of life for individuals and families coping with ALS, PBP, PLS and PMA. Elderwood Health Plan is a New York State-approved Managed Long-Term Care (MLTC) plan. It provides long-term care services to residents of Erie, Niagara, Monroe, Orleans, Genesee or Wyoming counties who are 21 years of age or older. To qualify for enrollment, the applicant must be in need of community based long term care services for more than 120 days. Supportive Medical Partners – (SMP website: www.SupMedPartners.org) provides consultative services to physicians and palliative care (symptom management) to patients with serious illness and their families in several area hospitals, patient homes and long term care settings. SMP serves patients of all ages with serious illness as well as frail elderly patients with advanced progressive diseases. SMP’s mission is to help each patient navigate through the course of serious illness, regardless of patient location, changes in diagnosis, or medical care. The SMP team of doctors, nurse practitioners, nurses and social workers are specially trained in palliative care. 

Home Connections Palliative Care assists patients with serious illness in their home or assisted living residence. Home Connections provides home-based care and support to enhance the quality of life for the patient and caregiver. 24/7 on call nurse support, social work visits, frequent phone contact to monitor the patient’s condition and answer questions as well as trained volunteers for additional support are available.
